Please list any relevant medical conditions, allergies, dietary requirements, medications, or other information we should be aware of.

Photography & Video

Photos and videos may be taken during the event for church communication and promotional purposes, including social media, newsletters, and the church website.

Transport

I give permission for my child to travel to and from the Youth Alive Conference in transport organised by the church, including a bus or other approved vehicle driven by authorised adult leaders.

Meals

Meals will be provided throughout the event. I have advised the church of any allergies or dietary requirements listed above.

Spending Money

Participants may have the opportunity to purchase food, merchandise, or other items during the event.

Any spending money brought by participants is their own responsibility.

The church and its leaders are not responsible for lost, stolen, or damaged money or personal belongings.

Personal Property

Participants are responsible for looking after all personal belongings they bring to the event, including but not limited to:

• Mobile phones
• Wallets
• Jewellery
• Clothing
• Bags
• Electronic devices
• Money

The church, its volunteers, pastors, and leaders accept no responsibility for the loss, theft, or damage of any personal property.

Behaviour Expectations

All participants are expected to:

• Treat leaders, venue staff, and other attendees with respect.
• Follow the instructions of youth leaders at all times.
• Remain with the group unless given permission by a leader.
• Behave in a manner that reflects the values of our church.

Should a participant behave in a way that is unsafe, disruptive, or repeatedly disobedient, parents/guardians may be contacted and asked to arrange for their child to be collected.

Parent/Guardian Consent

I acknowledge that I have read and understood this permission form.

I give permission for my child to attend the Youth Alive Conference with the church youth group.

I understand that reasonable care will be taken by the church and its leaders to ensure the safety and wellbeing of participants. I acknowledge that participation in travel and youth activities involves inherent risks, and I accept those risks except where caused by negligence or unlawful conduct.

I authorise the leaders to seek emergency medical treatment for my child if necessary and understand that every reasonable effort will be made to contact me first.

I confirm that the medical information I have provided is accurate and complete to the best of my knowledge.
